Group adult foster care helps improve independent living for men and women living with HIV/AIDS or at elevated risk for HIV acquisition who have co-occurring psychiatric or medical disabilities.

The private property building has multiple single-room occupancy units with varying kinds of housing/voucher-based support. The Group Adult Foster Care Program office is where weekly groups are held and where care giver staff are located throughout the week.

Patients have access to concierge staff 24/7 in the building. Care giver access is generally Monday-Saturday with varying day time hours depending on the needs of the patients and other events in the building. Weekly nursing access is available on Sundays for at least 2 hours weekly. Nursing consultation is available throughout the week and patients have direct access to the nurse through telephone or through concierge/caregiver staff.  Service delivery is provided directly by Nurse Coordinator and two Home Health Aide caregivers who are JRI employees, and one LICSW who is a contracted employee.

Medicaid is the primary funder of Group Adult Foster Care Services.

Patients may be referred through multiple sources including: housing staff, concierge staff, property management staff, other patients, loved ones (family/friends etc.), as well as outside medical staff. Most often the patients are involved in other JRI Health services and are identified as requiring additional supports, and therefore referred to GAFC. For enrollment in the program a formal referral must be placed by the patient’s primary care and reviewed by the nursing coordinator for appropriateness.
